Daycare attendance sheet

A weekly sign-in and sign-out sheet for the clipboard by your door. Add the week and the children’s names, or leave it blank and print a stack.

Why keep a sign-in sheet

Most state licensing rules for family child care homes require a daily attendance record showing when each child arrived and left. Inspectors ask for it, and in an emergency it tells you exactly who is in the house. It also settles billing questions: late pick-up fees and extra days are easy to back up with a signed sheet.

How to use this sheet

  1. Print one sheet per week and put it by the door with a pen.
  2. At drop-off the parent writes the time and initials in the in half of the day; at pick-up, in the out half.
  3. File the sheets by month. Your state sets how long you must keep them, often one to three years.

Requirements differ by state, so check your licensing regulations for what the record must contain.

Can parents sign in on a phone instead?

In the Daycare Binder app you tap a child’s name to check them in or out and note who brought or collected them. The week prints as this same sheet with the times filled in.

Do I record children’s health information here?

No. An attendance sheet only needs names and times. Keep health and allergy records in your enrollment file, not on the sheet by the door.

What size is the sheet?

US Letter, portrait, one week per page, with up to 12 children.
